Appointments:
To schedule an appointment we will need the following:
• A referral from your primary doctor
• A pathology or blood count report indicating a diagnosis of cancer or blood disorder
• Operative reports of any major surgeries
• Recent x-ray films and x-ray reports

Office Hours:
Our offices are open Monday through Friday from 8:30 am to 5:00 pm. All patients are seen by appointment only. As a courtesy, please call if you are delayed or must cancel your appointment. When you arrive please sign in with the receptionist. We will get your vital signs and appropriate blood work, then escort you to an exam room.

Office Procedures:
You will be asked to complete a Patient Office Visit Information form. This is important information that will assist in addressing your concerns. Since your primary doctor may not always be available, you should be aware of your diagnosis, medication names, and the last day that you received treatment. All medications should be refilled during office visits. If you need a refill between visits, call during regular office hours before your medication runs out. Many medications cannot be called into your pharmacy. Calls about a family member’s condition or test results should be placed during regular office hours. Please leave a message and your call will be returned as quickly as possible. If you call with an emergency, please state so clearly, so that your call may be forwarded directly to a nurse or doctor.

Laboratory
Many routine laboratory tests necessary for your care are performed in our office laboratory, or sent to a reference lab for analysis. These lab tests are billed as part of your office visit. Occasionally, special blood tests may be requested, which will be sent to an outside reference lab and you will be billed by the lab. We will inform you of any of these special tests.

Family Conferences
Many of our patients face serious illnesses. For this reason, we encourage each patient to bring a designated family member to their office visits or conferences to participate in discussions of diagnosis, prognosis, and treatment plans. This process helps explain the patient’s condition to family members or supportive friends. We are unable to discuss a patient’s care with each concerned individual; indeed, the doctor-patient relationship prohibits such discussions without the patient’s express consent. Under special circumstances, a family conference can be arranged to help address all concerned family member’s questions.

Second Opinion
We support and often encourage second opinions. Your doctor may suggest that you obtain an opinion of a specialized treatment center or physician specializing in the treatment of your illness. If you wish to have a second opinion, please discuss this with your physician; he or she will be able to provide you with the names of experts available, and will facilitate making the appointment and the transfer of appropriate records. We are also available to provide second opinions for patients with cancer or blood disorders. If you wish to have a second opinion consultation, or to transfer your care to our practice, please call one of our office locations to schedule an appointment. To provide second opinion consultations we will need for you to bring or have sent to our office:
• Pathology reports and pathology slides for review
• Operative report of any recent major surgery
• X-ray films or copies of x-ray reports, CT scans, MRI scans, or nuclear medicine scans
• Summary or treatment flow sheets of any previous treatments

Fees and Insurance
Payment is expected at the time of service for all office visits. Upon request, our office staff will confirm your primary insurance coverage, and file claims with your primary insurance carrier for office services. Any balance due after insurance payments will be reflected on your monthly statement and will be due at the time of receipt. For those patients requiring additional assistance, our office manager will help you in the development of a payment plan and assist in filing your insurance as needed.

Please notify the office if you have a change in insurance, address, or phone number. It is important that the office has a copy of your current insurance, Medicare, or Medicaid card(s) so that your claims are filed appropriately. If your insurance plan requires a referral for office visits, a referral must be received prior to your visit.

We participate with most Preferred Provider Organizations (PPO) and Health Maintenance Organizations (HMO). Most of the plans require a copayment at the time of service. Due to the constantly changing status of these plans, our participation is always subject to change, and current participation is not a guarantee of continued acceptance of these plans. For this reason, please discuss any anticipated changes of insurance with your physician.

We will file primary insurance claims for any physician services provided to you by our practice during a hospitalization. You will be responsible for providing the office with all the information necessary to file your insurance claims.
